Job Overview – Nurse Practitioner or Physician Assistant (Assessment Specialist):
Compensation: $130,000/year
Location: Bronx, NY
Schedule: Monday to Friday, 4:00 PM – 12:00 AM
Our healthcare client is seeking a Nurse Practitioner or Physician Assistant for an evening-shift, in-office role based in the Bronx, NY. This position is ideal for an advanced practice provider experienced in medical assessments and urgent care. You’ll evaluate pediatric, adolescent, and adult patients in the Department of Homeless Services system, document clinical findings, and support public health initiatives for underserved populations.
Responsibilities as the Nurse Practitioner or Physician Assistant:
- Medical Assessment & Triage: Perform comprehensive health screenings for pediatric, adolescent, and adult patients entering the DHS system.
- Clinical Documentation: Record patient history, symptoms, and findings accurately using the CARES electronic medical system.
- Acute Care & Decision-Making: Address immediate medical needs and guide treatment decisions using evidence-based resources.
- Preventive Counseling: Educate patients on topics like NRT, SIDS prevention, and other public health strategies.
- Care Coordination: Collaborate with onsite providers and DHS staff to ensure safe, continuous care for vulnerable populations.
- Regulatory Compliance: Follow clinical protocols and ensure assessments meet internal and regulatory standards.
Qualifications for the Nurse Practitioner or Physician Assistant:
- Education: Master’s degree from an accredited Nurse Practitioner or Physician Assistant program required.
- Licensure: Active and unrestricted New York State Nurse Practitioner or Physician Assistant license required.
- Certification: National board certification (AANP, ANCC, or NCCPA) required, with BLS certification preferred.
- Experience: Minimum of 2 years of clinical experience in urgent care, family medicine, community health, or public health settings.
- Technical Proficiency: Proficient in EMR systems, including CARES, with basic knowledge of word processing and clinical tools.
- Skills & Attributes: Compassionate, culturally competent provider with excellent communication, triage, and decision-making skills.
- Language Requirement: Bilingual in English and Spanish preferred.
