Job Description
PURPOSE OF POSITION
The Psychiatric Nurse Practitioner will be a member of the interdisciplinary team, whose purpose is to evaluate, treat, and plan for the aftercare of patients in the CPEP and CFS.
AREAS OF RESPONSIBILITIES: CPEP
Summary Of Duties And Responsibilities
- Provide direct care to patient assigned to the team, including appropriate and timely care and documentation.
- Complete comprehensive and brief psychiatric evaluations assessment, with a focus on comprehensive risk assessment for all patients and re-assessment of patients in a timely fashion consistent with policy.
- Conduct appropriate outreach and relevant documentation as per policy.
- Complete treatment plan documentation consistent with policy and appropriate to patient's risk level.
- Collaborate with the other members of multidisciplinary team (Psychiatry attending, nursing, social work, PhD, and activity therapy) to ensure quality and safe care of all patients.
- Ensure appropriate use of de-escalation, and participate and provide leadership in behavioral codes
- Participate in psychiatric codes, medical codes until the Code Team arrives
- Work with all team members to help patient move through appropriate level of care
- Appropriate risk stratification and re-assessment of risk level
- Participate in discharge planning and including referrals to alternate levels of care and outpatient clinics
- Collaborate and coordinate with primary care and other medical specialists
- Participates and conduct tier reviews as per policy
- Work closely with the collaborative physician
- Participate in utilization review and management.
- Maintain focus on key strategic priorities including patient experience, access to care, increased market share, and improving financial stability
- Provide evaluations, diagnostic assessments, and treatment recommendations for patients with a wide range of mental health and substance abuse challenges in the CFS
- Utilize knowledge of psychiatric medications and psychotherapeutic techniques in treatment planning for patients in CFS.
- Provide pharmacological and psychotherapeutic interventions in CPEP/CFS
- Provide appropriate primary therapy coverage, including case management and charting.
- Participate in enhancing the therapeutic milieu.
- Provide psychoeducational interventions for patients and families in CPEP/CFS
- Work within required collaborative agreement with a physician.
- A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
- Current certification to practice as a Psychiatric Nurse Practitioner issued by the NYSED; and
- Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
