JOB SCOPE:

This is a clinical position requiring in-depth knowledge of mental health, DSM 5-TR diagnostic criteria, and skills in crisis intervention and stabilization. ESSENTIAL FUNCTIONS

• Conducts daily visits in the community, and subsequent progress notes and other required documentation in accordance with agency and OMH standards.
• Completes Wellness Plans, Comprehensive assessments, risk assessments and other documentation as required
• Completes individual psychotherapy with consumers using evidence-based treatment modalities.
• Participates in daily morning meeting, and informs ACT team in daily meetings and weekly clinical meetings around family issues and incorporate findings into service
• plans involve family and significant others in the consumer’s treatment with the ACT team and provide psycho-education and relapse prevention based on motivational interviewing and cognitive behavioral strategies during visits.
• Act as advocate and liaison for consumers in relationships with family, support systems, landlords and other community supports. Provide 24-hour crisis intervention on-call services on rotating basis.
• Performs other duties, consistent with the goals and objectives of the program, as directed by Team Leader.

SKILLS AND KNOWLEDGE:

• Experience understanding family systems based on diversity of cultural and ethnic backgrounds
• Experience understanding various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery.
• Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
• Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
• Ensure progress notes are completed within 24-hours of the delivery of the service and are available to be read in the daily organizational meeting.

EDUCATION AND EXPERIENCE:

• Master’s Degree required - LMHC, LMSW, LCSW, or equivalent in licensure in NYS
• At least one year of experience working in human services with individuals with serious mental illness and experience in work with families.
• Must be outcome oriented in a fast paced environment with deadlines.
