Responsibilities/Job Duties

Provide patient care as outlined in written policies approved by nursing and medical administration
Provide safe and appropriate medical care to all patients and visitors
Act as a liaison between nursing staff and medical directors
Perform comprehensive assessments of patients based on medical history
Identify, diagnose, and document chronic illnesses and/or diseases in patients
Provide medical follow-ups, tracking, and reporting of patient health
Refer patients to clinics or community resources when necessary
Prescribe medication safely and efficiently
Write discharge prescription orders on the day of discharge
Determine the effectiveness of the plan of care
Manage emergencies by determining the level of risk and initiating emergency care
Consult with physician as appropriate
Participate in performance improvement and research activities
Various administration tasks as needed
