Job Description
The Coordinator Manager assigned will collaborate with the medical team to effectively coordinate and facilitate patient focused care leading to patient's timely and successful completion of preoperative requirement prior to surgery. The surgical coordinator/patient navigator will ensure HIPAA compliance in the gathering, securing, inputting and maintenance of required database of related surgical
Summary of Duties and Responsibilities:
- At the time patients are identified and scheduled for surgery, the surgical coordinator/patient navigator will assist patients with fulfilling the doctor's pre-operative orders as well as other necessary pre-surgical requirements; primary examples include:
- Provide patients with information regarding their pre-surgical requirements (i.e. completed pre-surgery checklist); guide/assist patient with navigation of hospital's services.
- Utilize the hospital's prevailing electronic health record and scheduling systems (i.e. EPIC) to arrange and schedule all required pre-operative clearance appointments (i.e. medical, psych, nutrition, financial, SCC, etc.); coordinates scheduling to ensure completion of all pre-operative requirements. Within 30 days of scheduled surgery date.
- Document patient's appointments for blood test, EKG, chest x-ray and other diagnostic test as ordered by doctors within relevant procedure time frame; monitor/track status of completion of pre-operative test to ensure patient's readiness within timeframes that are consistent with deadlines associated with surgery date.
- Reviews health insurance information and ensures eligibility. Communicates with Finance team and provides supporting document. Updates information.
- Maintain up-to-date contact information for patients; confirm and remind patients of upcoming appointments.
- Coordinate transportation arrangements for medically justified surgery patients.
- Coordinating access for translator services to ensure appropriate communication is obtained during the patient's pre-surgical and surgical process as needed.
- Reminds patient of surgery date and provides instructions.
- Serve as liaison between the assigned medical/surgical specialty, other Bellevue Hospital services (i.e. Finance Perioperative Services, etc.) and the patient. Functions would include and not be limited to:
- Provide assistance to ensure resolution of medical, social and financial problems that impact on patients' ability to obtain needed surgery Communicates with health insurance plan.
- Direct patients to the appropriate areas where help can be obtained, when surgical coordinator/patient navigator is unable to provide needed assistance.
- Maintain constant communications and follow-up to ensure patient's satisfaction with hospitals pre-surgical and related services.
- Ensure that changes (i.e. cancellation of planned surgical cases) are communicated to all relevant parties within a timely fashion, monitor and track cancellations; follow-up to ensure case is appropriately rescheduled or closed.
- Establish and maintain records that will enable timely assessment and reporting of work/caseload details
- Provide cross-coverage for other services surgical coordinating duties as needed.
- Participate in hospital-wide and departmental specific performance improvement and patient safety initiative
- Other related duties as assigned by direct supervisor and/or service administrator such as cross-coverage in all surgical coordination services.
Minimum Qualifications
1. A Baccalaureate Degree from an accredited college or university in Public Health, Public Administration, Business Administration, Social Work, Psychology or Rehabilitation Counseling, and three (3) years of full-time experience in medically-oriented health care and medical support systems environment, two (2) years of which included experience in an administrative or supervisory capacity in such occupations as physical, occupational, inhalation, speech and audiology therapy or as Medical/Laboratory Technologist, Microbiologist (various specialties), Chemist (biochemist), Radiation and X-Ray Technician or three (3) years of responsible level experience in business management systems or general administration including two (2) years in a supervisory or administrative capacity.
2. A Masters Degree from an accredited college or university in the disciplines listed in (1), and two (2) years of full-time experience as listed in (1), plus one (1) year experience in an administrative or supervisory capacity in occupations as listed in (1); or,
3. A satisfactory equivalent combination of training, education and experience.
Note: Certification or licensure may be required.
