Job Title: Registered Nurse Behavioral – Bridge to Home

Department: Bridge to Home (BTH) – Nursing

Location: Manhattan NY

Job Type: Contract (Special Project)

Shift : Wednesday 08:00 pm- 0830 am and Saturdays -Sundays 0830am- 08:00 pm

Meal Break: 30 minutes

Position Summary

The Bridge to Home (BTH) program supports patients with serious mental illness (SMI) who are ready for hospital discharge but lack immediate stable housing. The RN will provide medical care, education, and support to these individuals across various settings. This role emphasizes compassionate, patient-centered care and collaboration with interdisciplinary teams to support patient recovery, stability, and reintegration into the community.

Key Responsibilities

• Assess patients via interviews, chart reviews, and referrals
• Administer medications and monitor patient responses
• Initiate nursing interventions and patient/family education
• Coordinate and respond to medical and psychiatric emergencies
  
  

Requirements

License: Active New York State RN License

Education: BSN

Certifications: AHA Basic Life Support (BLS) – Current and valid

Experience

• Preferred: Prior experience with patients experiencing homelessness, mental illness, substance use disorders, or justice involvement
• Desired: Wound care experience, naloxone (Narcan) administration, and education skills
