*Part-Time Nurse Practitioner (PMHNP/FNP) – Top NYC OPWDD Organization | $85–$90/hr | Flexible 2 Days/Week*
- Location:* Bronx, NY
- Pay:* $85–$90/hr
- Schedule:* 2 days/week – you choose the days
Nurse Practitioner Key Responsibilities
- *Conduct Comprehensive Evaluations:* Perform physicals and health assessments for new and existing participants.
- *Provide Acute and Chronic Care Management:* Address new health concerns, manage chronic conditions, and collaborate with interdisciplinary teams.
- *Coordinate Referrals and Diagnostics:* Order and review lab tests, imaging, and specialist referrals as needed.
- *Prescribe and Adjust Treatment Plans:* Manage medications, therapeutic diets, and individual care plans.
- *Lead Preventive Health Efforts:* Deliver preventive services, monitor health indicators, and support infection control initiatives.
- Active Nurse Practitioner (NP) license in New York
- Minimum of 2 years of NP experience
- Board certification as a PMHNP or FNP
