Job Details

We are seeking a dedicated and empathetic Social Worker to support the residents of the Penn South community. In this role, you’ll help individuals navigate life’s challenges, access resources, and maintain a high quality of life in a supportive housing environment.

Key Responsibilities

Conduct psychosocial assessments and develop individualized care plans

Provide counseling, advocacy, and crisis intervention services

Connect residents with appropriate social, health, and community resources

Monitor and follow up on service plans to ensure needs are met

Collaborate with housing staff and external providers

Maintain accurate case records and documentation
